Patients who are dying of cancer usually give up eating and then stop drinking. This raises ethical dilemmas about providing nutritional support and fluid replacement. The decision-making process should be based on a knowledge of the risks and benefits of giving or withholding treatments. There is no clear evidence that increased nutritional support or fluid therapy alters comfort, mental status or survival of patients who are dying. Rarely, subcutaneous fluid administration in the dying patient may be justified if the family (...) remain distressed despite due consideration of the lack of medical benefit versus the risks. Some cancer patients who are not imminently dying become dehydrated from reversible conditions such as hypercalcaemia. This may mimic the effects of advanced cancer. These conditions should be sought and fluid replacement therapy should be given along with the specific treatments for the condition. (shrink)

Political liberals ask citizens not to appeal to certain considerations, including religious and philosophical convictions, in political deliberation. We argue that political liberals must include a demanding requirement of intellectual modesty in their ideal of citizenship in order to motivate this deliberative restraint. The requirement calls on each citizen to believe that the best reasoners disagree about the considerations that she is barred from appealing to. Along the way, we clarify how requirements of intellectual modesty relate to moral reasons for (...) deliberative restraint. And we argue against attempts to weaken our requirement of intellectual modesty by emphasizing those moral reasons. (shrink)

Compact quantum systems have underlying compact kinematical Lie algebras, in contrast to familiar noncompact quantum systems built on the Weyl-Heisenberg algebra. Pauli asked in the latter case: to what extent does knowledge of the probability distributions in coordinate and momentum space determine the state vector? The analogous question for compact quantum systems is raised, and some preliminary results are obtained.

In Responding to Global Poverty, Christian Barry and Gerhard Øverland argue that, while exploitation is morally problematic, responsibilities not to exploit are characteristically less stringent than responsibilities not to harm. They even suggest that exploiters’ responsibilities to assist the exploited may be weaker than the responsibilities of culpable bystanders who are able to help the poor but fail to do so We think Barry and Øverland underestimate the prospects of the exploitation argument. In our paper, we suggest that exploitation can (...) plausibly be understood as a kind of harm. If exploitation harms, then it requires special justification and can generate stringent responsibilities not to exploit that have a different ground than those generated by morally culpable failures to assist. This suggests an important way to rehabilitate arguments for poverty relief on the basis of a duty not to harm, and that there is more interesting territory to explore than Barry and Øverland’s arguments suggest. There has been a long tradition of characterizing man as the animal that talks. However, the remarkable ability of using pictures also only belongs to human beings, after all we know empirically so far. Are there conceptual reasons for that coincidence? Such a question belongs to the philosophy of language just as well as to philosophical visualistics. Comparing the two abilities to use words or pictures yields several similarities as well as distinctions. A well-known conceptual disparity between pictures and words (...) appears in their relation to perception: the difference can be further determined in an act-theoretic manner by four modes of use of the sign vehicles during the corresponding sign acts. Furthermore, the figure/ground dichotomy means something different for language uses and picture uses. In both cases, however, there is a close relation to the function of context building, by which humans are able to communicate not only with respect to the present situation of behavior but with respect to arbitrary contexts as well. Although the structural comparison does clarify the conceptual relations, it cannot explain that the conceptual structure ought to be like that. Therefore, the paper concludes with the programme of a "concept-genetic" consideration of the two abilities (i.e., to use propositional language or to use pictures) that is able to give us such a foundation. Among young liberal Catholic intellectuals, Lonergan is held in extremely high esteem. His philosophic treatise, Insight, is considered to be the important philosophic book where Thomism genuinely encounters contemporary secular philosophy. But outside this circle of Catholic intellectuals Lonergan's thought is barely known. This collection of articles does reflect the comprehensiveness and depth of his thought. Papers range over intricate theological discussions of the Assumption, Christ, marriage, the role of a Catholic university in the modern world, and technical philosophic issues (...) such as the form of inference and geometric possibility. Because the papers are short and have been written for a variety of audiences, it is difficult to discern any overreaching continuity and perspective. Many of the discussions demand a more thorough and critical analysis than is exhibited here. The introduction by Crowe is helpful for reconstructing Lonergan's intellectual development and supplying a broader context for appreciating the papers, although the tone is more reverential than critical.—R. R. J. Hankinson traces the history of ancient Greek thinking about causation and explanation, from its earliest beginnings through more than a thousand years to the middle of the first millennium of the Christian era. He examines ways in which the Ancient Greeks dealt with questions about how and why things happen as and when they do, about the basic constitution and structure of things, about function and purpose, laws of nature, chance, coincidence, and responsibility.

Background: When a genetic mutation is identified in a family member, internationally, it is usually the proband’s or another responsible family member’s role to disclose the information to at-risk relatives. However, both active and passive non-disclosure in families occurs: choosing not to communicate the information or failing to communicate the information despite intention to do so, respectively. The ethical obligations to prevent harm to at-risk relatives and promote the duty of care by genetic health professionals is in conflict with Privacy (...) laws and professional regulations that prohibits disclosure of information to a third party without the consent of the proband. In New South Wales, Australia, amendments to Privacy legislation permits such disclosure to living genetic relatives with the process defined under guidelines although there is no legal duty to warn. This study assessed NSW GHP’s awareness and experience of the legislation and guidelines. Methods: An online survey collected demographics; theoretical knowledge; clinical scenarios to assess application knowledge; attitudes; confidence; experience with active non-disclosure. A link to correct answers was provided after completion. Knowledge scores above the median for non-parametric data or above the mean for parametric data were classified as ‘good’ or ‘poor’. Chi square tests assessed associations between confidence and knowledge scores. Results: While many of the 37 participants reported reading the guidelines, there was limited awareness of their scope and clinical application; that there is no legal duty to warn; and that the threat does not need to be imminent to warrant disclosure. No association between confidence and ‘good’ theoretical or applied clinical knowledge was identified. Uncertainty of their professional responsibility was identified and in the several case examples of active non-disclosure that were reported this uncertainty reflected the need for further understanding of the guidelines in regard to the processes required before disclosure was initiated. Conclusions: There is a need for further education and training about the guidelines associated with the legislation that would be relevant to support disclosure. The findings may inform future strategies to support introduction of policy changes in other jurisdictions where similar regulatory regimes are introduced. (shrink)
